Earlier, World No 1 Novak Djokovic had beaten Milos Raonic 7-6 (7/1), 6-2, 6-2 to level the tie at 2-2.
Serbia, the 2010 champions, will host the November 15-17 final against the Czechs who beat Argentina 3-2 in their semi-final in Prague.
