The problem
Issues come when women who need necessary care for delivery, abortion, excessive bleeding, fibroids, painful periods, emergencies or even cancer.&nbsp;However either patients or&nbsp;doctor&nbsp;who were scared to treat women patients&nbsp;for imaginary&nbsp;high risk of getting Covid made more women patients not&nbsp;attended&nbsp;adequately. Few Gynaecologists were giving t care as needed with due precaution &amp; proper use of&nbsp;RTPCR, Antigen/ Antibody tests of their own staff plus all patients who needed admission in their hospital.
What Consultant should know in Covid times ?
Whether to do open or Laparoscopic surgery? Without any documented evidence of aerosolization or risks of spread by the route of surgery was an issue. Dedicated well known Gynaecological Laparoscopic Surgeon Dr. Prakash Trivedi &amp; Dr. Soumil Trivedi, Laparoscopic &amp; Robotic surgeon took up the serious task of inventing, innovating &amp; designing a method for safe Laparoscopic surgery needed&nbsp;&nbsp;for women&rsquo;s problems in emergency &amp; other surgery which need not wait.
In the Operation theatre
At Dr. Trivedi&rsquo;s Total Health Care Centre developed a &nbsp;safe administration of anaesthesia for the said surgery with proper PPE kit worn by the anaesthetist. The anaesthesia medicines &amp; gases were delivered to the patient through a specialized closed circuit machine with use of specialized Heat Moisture Exchanger &ndash; bacterial &amp; viral (HME-BV) filters &amp; disposable tubings filtering both the gases going into &amp; that coming out or exhaled by the patient.
The surgical team with proper PPE kits enter the surgical theatre for surgery. Laparoscopic surgery requires use of medical gas carbon dioxide for successful performance of the surgery which was also delivered to the patient via an insufflator machine through HME-BV filter &amp; special tubing.
The required surgery be it for uterus removal- hysterectomy, fibroid removal- myomectomy, surgery for ovarian cyst, endometriosis, prolapse or looseness or sagging of uterus &amp; vagina, for patients keen to conceive or surgery for early gynaecological cancers of uterus, cervix, Mesh for Incisional hernia etc was carried out as usual with due precautions. During the surgery use of certain energies can generate smoke which needs to come out. The gases that go in need to come out. The blood or bodily fluid needs to be sucked out from the patient&rsquo;s body. This is where a huge breakthrough was made by Dr. Trivedi &amp; his team designed and used a Double Closed Circuit Suction Assembly capable of removing the blood or other bodily fluids as well as the smoke generated&nbsp; &amp; gas after completion of the surgery in a safe manner going straight into a powerful disinfecting solution into 2 specially designed bottles through specialized tubings &amp; HME-BV filters inactivating any bacterial or viral particles.
Hence at no stage any gas, smoke, blood or body fluid can come in contact with air of operation theatre protecting Surgeon, Assistant, Sister, Anaesthetist, , OT staff, machines and above all the patients, making it safe.&nbsp;
After the completion of surgery everything used were safely disposed. All patients are followed for 14 days for any Covid symptoms.
From March 2020 till date at Dr.Trivedi&rsquo;s Total Health Care Centre, Ghatkopar East, a large number of Laparoscopic surgery &nbsp;were done.Internationally reputed &nbsp;Speciality Journal has published this important work to spread this new technique.&nbsp;
Such innovation could be very useful during Pandemic times &amp; later also for safety to all in Laparoscopic surgery not women but men also.
